San Carlos Apache Tribe Enrollment Department Launches Vital Records Outreach Program

San Carlos, AZ — January 6, 2025 — The San Carlos Apache Tribe Enrollment Department is excited to announce a partnership with the Gila County Office of Vital Records to bring vital records services closer to tribal members. This outreach program will provide essential services at the Tribal Enrollment Office starting January 2024.

Important Vital Records Outreach Update:
The Gila County Office of Vital Records will conduct its first outreach services on Jan. 7th and Jan. 14th, with a staff member available from 8:00 a.m. to 4:00 p.m., including a one-hour lunch break. Appointments are not required.

Services offered will include:

  • Applications for vital records
  • Acknowledgment of Paternity (AOP) forms
  • Affidavits to Correct or Amend certificates

Tribal members are advised to bring the following:

  • A valid ID or driver’s license (Tribal IDs accepted)
  • A form of payment (cash, card, or check accepted)
  • Any supporting documents needed

Fee Schedule:

  • Certified copies: $20 each
  • Changes to certificates: $30 ($20 certified copy fee + $10 change fee)

Please note:
Certificates will not be printed during the outreach services. Completed certificates will be delivered at the next scheduled outreach date (e.g., applications submitted on Jan. 7th will be delivered on Jan. 14th).
