The San Carlos Apache Tribe Enrollment Department

Tribal Enrollment Department is a program component to assist San Carlos Apache in the compilation and documentation of Tribal Rolls; also, to assist in planning, maintain and enhancing the Tribe’s enrollment data system. To serve as a basis of distribution of judgment funds and other Tribal funds and to provide Administrative support; to other enrollment/membership related purposes.

Services

Tribal Enrollment offers statistical information to tribal members, departments, programs, agencies; offers handicap accessible and with accommodations provided; also offers the following services:
● CIB (Certificate of Indian Blood)
● Tribal ID (Adult/Minor)
● BIA Form (Indian Preference) 4432
● Completion of Eagle Permit Application Forms
● Relinquishment (Adult/Minor) Forms
● Application for Enrollment
● Vital Records Copies
● Offer Consent/ Release Forms
● Ancestral Trace/ Family trees
● Address Change Forms
● Conduct Monthly Enrollment Committee Meetings
