San Carlos Apache Tribe Enrollment Process
Welcome to the San Carlos Apache Tribe’s Enrollment Process, a significant journey that connects individuals to the rich tapestry of our tribal heritage. Our Tribal Enrollment Department is dedicated to facilitating this process with transparency and efficiency. To become an enrolled member of the San Carlos Apache Tribe, certain documents are required to ensure accuracy and compliance with our Enrollment Code. Below, we outline the essential steps and necessary documentation for enrollment. Documents Required for Enrollment

Original Enrollment Application – use black or blue ink pen to complete
This application shall be signed by a parent, custodial parent, or legal guardian.
(Pursuant to the Enrollment Code – Chapter V, Sec. 500 C. 12) - 2
Certified Copy or Original State Issued Birth Certificate –
Xerox copies or Hospital heirloom (Footprints) BC will not be accepted.
(Pursuant to the Enrollment Code – Chapter V, Sec. 500, D. 2)Original Birth Certificates become the property of the San Carlos Apache Tribe Enrollment Department.

Paternity –
A child born to an unwed mother can legally claim only half (1/2) of the mother’s blood degree, excludes the father’s blood degree unless acceptable proof of paternity is submitted.Paternity must be established if the applicant’s mother is a non-member or non-Indian.

Custody Orders/Guardianship –
This document shall be signed by a Judge to verify that the guardian is legally authorized to act on behalf of the applicant. Power of Attorney will not be accepted. - 5
If applicable, Non-member Verification –
Verification from non-member parent’s Tribe/Nation stating child/applicant is not enrolled with that Tribe/Nation or attempting to enroll there. The San Carlos Apache Tribe prohibits Dual-Enrollment.

Return Application with the required documents to the Tribal Enrollment Department. Tribal Enrollment Department Office hours are Monday through Friday, 8 am to 4:30 pm.
