To apply for a Certificate of Indian Blood you must be enrolled in the San Carlos Apache Tribe. Click the link below and complete the Authorization to Release form, check the “Certificate of Indian Blood (CIB)” box and fill out remaining information. FORM MUST BE NOTARIZED and delivered either by mail or in person to the Enrollment Department. Please be advise of the fees for copies of documents requested.
